4. Educational qualification(s)
5. Birth certificate or age declaration
6. Local govt. Identification letter
7. Current practicing license (where applicable)
8. Acceptance letter
9. Medical certificate of fitness
10. Personnel record of service
11.  Two passport size Photograph

Medicals commences on the 16th August at the NDUTH.
